Our office sees primarily three types of patients:
At the time of the initial examination it is required that any previous diagnostic tests performed including x-ray films, MRI films, CT scan films, etc. are brought to the appointment for the doctor's review. If these tests are not brought to the appointment it may be necessary to reschedule the appointment.
For your convenience, we will bill your insurance company and collect payment. Any portion of the fee not covered by your insurance company will be billed to you. You will receive monthly bills including the amount billed to your insurance company, until the balance is paid.
Arrangements regarding your insurance plan and benefits to which your are entitled are between you and your insurance carrier. Full payment of your bill is your responsibility.
Our office is a participating provider with Medicare. We will bill you for your deductible and co-payments after Medicare pays. Please bring your card for Medicare and any supplemental policies.
We will fill out your disability forms. In order to research the required information and complete the form, we require a $15 processing fee and five business days.
If you are uninsured or do not provide evidence of insurance, please be prepared to pay at the time of service. If insured, you will be required to pay any deductible or co-payment at the time of service.
